Once your appointment is scheduled, you will be emailed a set of medical forms to complete.  These medical forms must be completed prior to your first appointment.  The forms include an in-depth, nineteen page questionnaire about your medical history, current health issues and regimen for our providers.  This is a unique tool that helps our medical staff assess what your symptoms are telling us.  It allows our providers to create a preliminary treatment plan, and be completely prepared for  your appointment.
