Prescription Refill

The best course of action is to contact your pharmacy which will fax us the request for authorization. If they are unable to assist you please take into consideration our office hours when requesting prescription refills. We will authorize your refills as long as you are not past due for an appointment.

In order to assist everyone in an efficient manner, we ask that you submit a specific request to us only once, either by phone, email or web request. This will avoid delays in addressing your concerns and allow us to return your inquiry promptly.
