
What if Alzheimer’s doesn’t begin with memory loss?
What if the disease actually starts 10, 15, or even 20 years before the first forgotten name or misplaced keys?
That is exactly what many researchers now believe.
Brain scans can detect declining brain energy metabolism years before symptoms appear, suggesting that Alzheimer’s may begin long before memory fades. This emerging understanding is changing how scientists think about one of medicine’s most devastating diseases—and may eventually change how we protect our brains long before symptoms appear.
For years, Alzheimer’s research focused primarily on removing amyloid plaques from the brain. While newer medications can reduce these plaques, the improvements in memory and daily functioning have generally been modest, and none has been shown to stop or reverse the disease. That has prompted many researchers to ask a different question:
What if amyloid is only part of the story?
When the Brain Runs Low on Energy
One of the biggest shifts in Alzheimer’s research is the growing recognition that the disease may be driven as much by impaired brain metabolism as by amyloid itself. In many people, the brain gradually loses its ability to use glucose efficiently, creating an energy crisis years before symptoms appear.
Although the brain makes up only about 2% of our body weight, it uses nearly 20% of our daily energy. Healthy brain cells depend on glucose, healthy mitochondria (the tiny “power plants” inside our cells), oxygen, and nutrients to function. When insulin resistance, chronic inflammation, poor diet, lack of exercise, vascular disease, poor sleep, or other stressors interfere with this process, brain cells struggle to produce the energy they need.
This concept has led some researchers to describe Alzheimer’s as “Type 3 diabetes”—not as an official diagnosis, but as a way of illustrating the important role that insulin resistance and metabolic health may play in cognitive decline.
Another exciting area of research involves ketones, the brain’s “backup fuel.” Unlike glucose, ketones can often be used efficiently even when the brain has become resistant to insulin. This is one reason scientists are studying whether regular exercise, time-restricted eating or intermittent fasting (when medically appropriate), and carefully designed ketogenic nutrition may help support cognitive function. While these approaches are not cures and more research is needed, they reflect a growing focus on improving the brain’s energy supply—not simply removing plaques.
A Different Way to Think About Alzheimer’s
Amyloid plaques and tau tangles remain important features of Alzheimer’s disease, but they may represent just one part of a much larger picture. Researchers are increasingly studying how chronic inflammation, metabolic dysfunction, cardiovascular health, genetics, environmental exposures, sleep, and lifestyle interact over decades to influence brain health.
This broader understanding aligns well with a functional medicine approach.
Rather than asking only, “What drug treats Alzheimer’s?”, functional medicine asks, “Why did this brain become vulnerable?”
Instead of focusing on a single pathway, practitioners work to identify and address factors that may be affecting brain function, including blood sugar balance, inflammation, infections, toxins, poor oral/dental health, nutrition, sleep quality, exercise, cardiovascular health, gut health, and other modifiable lifestyle factors. The goal is not simply to manage disease after it appears, but to support a healthier brain long before symptoms develop.
Hope for the Future
One encouraging finding has emerged consistently from recent research: many of the factors associated with Alzheimer’s are modifiable.
The 2024 Lancet Commission concluded that addressing risk factors such as diabetes, hypertension, obesity, hearing loss, physical inactivity, depression, social isolation, and poor sleep could significantly reduce the number of future dementia cases.
