Semaglutide vs. Tirzepatide or Retatrutide for Weight Loss

Weight loss medications have changed significantly in recent years. Semaglutide and tirzepatide are now established options for chronic weight management, while retatrutide is an investigational medication that is generating interest because it targets three hormone receptors rather than one or two. 

If you’re comparing semaglutide vs. tirzepatide for weight loss and wondering where retatrutide fits in, the biggest differences involve how each medication works, the weight loss demonstrated in clinical trials, side effects, availability, and FDA approval status. While all three influence pathways involved in appetite and metabolism, they are not interchangeable, and Retatrutide may be available, but is not yet an FDA-approved treatment. 

How Do Semaglutide, Tirzepatide, and Retatrutide Work?

Understanding the semaglutide vs. tirzepatide differences starts with the hormones each medication targets. Semaglutide is a GLP-1 receptor agonist. GLP-1 is a naturally occurring hormone involved in appetite, blood sugar regulation, and digestion. By mimicking its activity, semaglutide can decrease appetite, increase feelings of fullness, and slow gastric emptying. 

Tirzepatide works on two pathways: GLP-1 and glucose-dependent insulinotropic polypeptide (GIP). If you are wondering how tirzepatide works vs. semaglutide, this additional GIP activity is the primary distinction. Both medications affect GLP-1 receptors, but tirzepatide adds a second hormone pathway involved in metabolic regulation. 

Retatrutide goes another step further. This investigational medication targets GLP-1 and GIP receptors along with glucagon receptors, making it a triple hormone receptor agonist. As of August 2026, retatrutide remains investigational and is not approved by the FDA or any other regulatory agency. It is being evaluated in Phase 3 clinical trials.

These distinctions are important when making a semaglutide vs. tirzepatide comparison. Retatrutide may eventually expand the available options, but it isn’t a widely available option just yet. 

Semaglutide vs. Tirzepatide Weight Loss Results

One of the biggest questions surrounding tirzepatide vs. semaglutide is which medication produces greater weight loss. The STEP 1 trial evaluated once-weekly 2.4 mg semaglutide in adults with overweight or obesity without diabetes. Participants receiving semaglutide lost an average of 14.9% of their initial body weight over 68 weeks, compared to 2.4% among participants receiving placebo.

Tirzepatide has produced larger average reductions in some trials. In SURMOUNT-1, participants without diabetes lost an average of 15.0%, 19.5%, or 20.9% of their initial body weight at 72 weeks, depending on their tirzepatide dose. 

More importantly, for the tirzepatide vs. semaglutide weight loss effectiveness comparison, researchers have now studied the medications head-to-head. The 2025 SURMOUNT-5 trial found an average weight reduction of 20.2% with tirzepatide and 13.7% with semaglutide after 72 weeks among adults with obesity but without diabetes. 

That does not mean tirzepatide will produce greater weight loss for every individual. Medication tolerance, dose, medical history, lifestyle, adherence, and individual metabolic factors all affect results.

How Does Retatrutide Compare for Weight Loss?

Retatrutide has attracted considerable attention because of the weight reductions observed in clinical trials. However, comparisons with semaglutide and tirzepatide require caution because results from separate studies are not the same as a direct head-to-head comparison. In May 2026, Eli Lilly reported topline results from the Phase 3 TRIUMPH-1 trial. Adults without diabetes receiving the highest 12 mg retatrutide dose lost an average of 28.3% of their body weight at 80 weeks. Participants with a baseline BMI of at least 35 who continued into a study extension experienced average weight loss of up to 30.3% at 104 weeks. 

Additional Phase 3 results announced in July 2026 found average weight reductions of up to 20.8% among participants with obesity or overweight and type 2 diabetes and up to 22.6% among adults with severe obesity and established cardiovascular disease. These results are promising, but retatrutide remains under investigation. 

Tirzepatide vs. Semaglutide Side Effects

When comparing the side effects of tirzepatide vs. semaglutide, there is significant overlap. Because both affect GLP-1 signaling and slow digestion, gastrointestinal symptoms are common, particularly while the dose is being increased. Potential side effects can include:

  • Nausea
  • Vomiting
  • Diarrhea
  • Constipation
  • Abdominal discomfort

In the direct SURMOUNT-5 comparison, gastrointestinal events were the most common side effects with both medications and were generally mild to moderate. So, semaglutide vs. tirzepatide: which is safer? There is no universal answer. Both have established safety profiles and FDA-approved indications, but individual health history matters. A medical provider should review contraindications, other medications, existing health conditions, and potential risks before recommending either option. Retatrutide cannot yet be given the same established safety comparison because its safety profile is still being evaluated in clinical trials.

Semaglutide Injection vs. Tirzepatide Injection

For patients considering injectable treatment, semaglutide and tirzepatide have an important similarity: FDA-approved injectable versions used for weight management are administered once weekly. The more meaningful difference in semaglutide injection vs. tirzepatide injection is therefore not injection frequency but the medication itself. Semaglutide targets GLP-1, whereas tirzepatide targets GLP-1 and GIP. 

Treatment typically begins at a lower dose that is gradually increased. This titration process gives the body time to adjust and may help manage gastrointestinal side effects.

Tirzepatide vs. Semaglutide Cost

The tirzepatide vs. semaglutide cost depends on the prescribed product, insurance coverage, eligibility for manufacturer savings programs, dosage, and pharmacy. Coverage for weight loss medication also varies considerably among insurance plans. Rather than choosing a medication based solely on advertised pricing, patients should consider the complete treatment plan. This is because the cost increases considerably as the dose does, and gradually increasing the dose is important to continue losing weight.  Cost, expected benefits, side effects, health history, long-term treatment needs, and insurance coverage all matter.

Which Weight Loss Medication Is Right for You?

Looking solely at semaglutide vs. tirzepatide weight loss percentages does not tell you which medication is best for your body. The appropriate treatment depends on your medical history, current weight, metabolic health, medications, previous weight loss attempts, treatment goals, and ability to tolerate side effects. Semaglutide has an extensive body of clinical evidence and targets GLP-1. Tirzepatide targets both GLP-1 and GIP and produced greater average weight loss than semaglutide in a direct obesity trial. Retatrutide targets GLP-1, GIP, and glucagon and has produced substantial weight reductions in Phase 3 research, but it remains investigational.
